Each Family will be Required to Support Our Fund Raising Raffle

By Selling $100 in Raffle Tickets.

Raffle Tickets will be distributed at the Parent’s Meeting in June,

· Proof of Residency - Driver’s License, Utility Bill. Etc. (New Participants Only)

· Original & One Copy of Birth Certificate (New Participants Only)

· Proof of Medical Insurance

· Recent Wallet Size Photo (2in x 2 in)

|
· Two Checks or Money Orders Payable to Greater Freehold Area Pop Warner – one for the Registration Fee(s) and one for the Volunteer Work Bond(s)
